Accessing a primary care provider can be challenging for many residents, and we recognize the impact that difficulty accessing care can have on health and well-being.
Looking for a family doctor or nurse practitioner?
Ontario’s Health Care Connect program is a free service that helps residents find a family doctor, nurse practitioner or specialist who is accepting new patients in their area.
How it works:
- Register online or call 811 to join the program.
- A nurse, known as a care connector, reviews your request.
- Your care connector searches for an available primary care provider in your area.
- When a match is found, your care connector will contact you as soon as possible.
A family doctor or nurse practitioner can help diagnose and treat common illnesses, manage chronic conditions, provide referrals to specialists, and support your overall health.
